The City of Whittlesea received funding from the Department of Human Services in 2008 to develop new partnering arrangements between agencies that provide Home and Community Care Services to residents in need of support to remain in thier own homes. The attached documents include a report on the outcomes of the project and a copy of the Assessment Protocols developed.
