Chronic Disease Self Management Interest GroupThe Chronic Disease Self-Management Special Interest Group was formed in February, 2004. Its membership is health professionals and consumers in Victoria who wish to promote self-management programs as means to improve the health and quality of life of all people with chronic illnesses. Membership is open to all health professionals and consumers who would like to share their work, or learn more, in this area. Many of the members of the Special Interest Group run self-management programs as part of the broader programs they are developing under the Hospitals Admissions Risk Program (HARP) and Integrated Disease Management (IDM) programs. Both HARP and IDM are funded though the Department of Human Services. |
