Chronic Illness Care Systems Development
Community Health has been providing care for people with chronic health conditions since its inception. The increasing prevalence of chronic health problems in the community and increasing financial burden of providing care within hospitals has resulted in greated demand for care within a community health setting.
The four community health services in the NCMPCP catchment: North Richmond, North Yarra, Darebin and Plenty Valley commisioned this report with NCMPCP to develop a Chronic Illness Care Model for Community Health.

An evaluation framework for the early intervention in chronic disease in Community Health services initiative
In February 2006 the Australian Institute of Primary Care (AIPC) at La Trobe University was engaged by DHS to develop a generic evaluation framework for use within the EIiCD initiative according to an agreed list of evaluation questions. At that time, the nine selected community health services were in various stages of developing individual programs in conjunction with other partners (such as the local division of General Practice (DGP) and Primary Care Partnership (PCP)). THis report provides an overview of the individual programs established under the initiative, the refinement of the evaluation questions and program logic, the development of the evaluation framework and the subsequent consultation process undertaken, as well as detailing the proposed final evaluation tools for use as an ongoing component of the ELiCD initiative.

improving quality of life.
• The recent 2006–07 federal budget allocated an
unprecedented $515 million over 5 years for activation of
patient self-management activities, commencing this financial
year.
• Previous attempts in other countries to incorporate selfmanagement

Engaging Older CALD People in Physical Activity Forum
Engaging Older CALD People in Physical Activity is the second forum offered by the Integrated Health Promotion committee of the North Central Metro Primary Care Partnership (NCMPCP).
The forum aimed to share learnings from work taking place across the NCMPCP municipalities of Whittlesea, Darebin and Yarra. Presentations provided an opportunity to showcase the work from the three municipalities and learn from each others experiences.
Each of the municipalities in the north central metro catchment records an increasing number of older people from culturally and linguistically diverse (CALD) backgrounds. These communities are also often over represented in the statistics for chronic disease. There is evidence for maintaining levels of physical activity throughout the life cycle as a preventive measure in developing chronic disease and for other social and mental health benefits.
Encouraging CALD people, particularly older members of the community, to improve their levels of physical activity has proven to be difficult. This forum presented some of the work undertaken and highlighted some common indicators to successfully working with the older CALD community.

Interagency Care Planning Protocol - Pilot Project Report Nov 2005
A North and West Primary Care Partnership Alliance Service Coordination Project
This project was the first step in ongoing development of the Care Planning component of Service Coordination in the North West Region. The project objectives included the development of practices and protocols around multi-agency Care Planning, including the operationalising of the Service Coordination Plan, and the role of the key worker in the process. A wide range of service providers were involved across four PCPs in the North West Region (Moonee Valley Melbourne, Brimbank-Melton, Westbay and Banyule Nillumbik Primary Care Alliance).

NCMPCP eReferral Audit 2007
North Central Metro Primary Care Partnership eReferral Audit 2007 was
conducted with core member agencies to determine the barriers and enablers
associated with the implementation and use of electronic referral systems. An
examination of organisational procedures was carried out to identify ways

As a requirement for funding PCP's are to provide a Community Health Plan for the period 2006 - 2009 with annual updates as required and annual Community Health Plan Implementation Agreements (CHPIA) dule July 2007 and 2008 with the final CHPIA due July 2009. Victorian Auditor-General's Report Tabled:
A report by Auditor-General, Des Pearson, titled "Promoting Better Health Through Healthy Eating and Physical Activity" was tabled in the Victorian Parliament on 20 June 2007. This report examined whether the Victorian Government’s investment in health promotion had 'encouraged Victorians to adopt healthier eating habits, to exercise more regularly, and to achieve healthy weight levels'. What can we do to ensure access to nutritious food for all?
Food security is becoming an important issue in Australia. A brief overview of the NCMPCP Food for the Future forum presentations highlighting some important issues and solutions is attached.
Overall the forum rated well. Participants indicated that information covered by the program will help raise awareness of food security issues, provide direction for needs analysis and in developing policy. There was also interest in developing and contributing to networks.
An on-going role for the PCPs in supporting food security issues in the region could be considered. This could include networking and information sharing between agencies
