The North Central Metro Primary Care Partnership’s vision is for coordination and leadership in integrating health promotion throughout the catchment, advocating for and applying a social model for health in service planning and provision.
In Victoria, the term ‘integrated health promotion’ refers to agencies and organisations from a wide range of sectors and communities in a catchment working in a collaborative manner. A mix of health promotion interventions and capacity building strategies are used to address priority health and well-being issues.
The North Central Metro Primary Care Partnership provides leadership and strategic direction for promoting healthy lifestyles across the municipalities of Yarra, Darebin and Whittlesea. An Integrated Health Promotion steering group set up in 2005 supports planning within key health services and local government in the catchment area. To maximise the use of available health promotion resources there are agreed priority target groups and health promoting activities.
The North Central Metro Primary Care Partnership is committed to addressing the health needs the Aboriginal and Torres Strait Islander community.
